Boarding Check In and Doctor Exam

Once this form is completed, it will be emailed to our office; nothing to print! Once the office receives your boarding deposit and check in form, your reservation will be confirmed and your check-in will be much quicker. If we receive your form 48 hours prior to check-in, you will receive $10.00 credit on your boarding services. Pets needing an exam or services with the vet, please plan for 10 to 15 minutes to speak with a member of our medical staff about your pet’s healthcare.

No need to bring bedding, we include bedding in our boarding services. If you would like to bring bedding that can be donated to our borders after your pet’s stay, that is always appreciated!

Check-in times are Monday through Friday 7:30 a.m to 5:30 p.m. and Saturdays 7:30 a.m to 11:30 a.m. Please notify the office if your pet needs to be dropped off within 30 minutes of closing time.

Please note, if your pet will be receiving a bath before check out, bath days are Mondays, Wednesdays, and Fridays. Your pet will be ready for pick up After 3:00 p.m.

  • East Marietta Animal Hospital reserves the right to bathe any pet that soils itself, is infested with fleas or ticks or has a strong "doggy" odor. Any fees incurred from bathing are the responsibility of the client and must be paid for at the time of discharge. There are no exceptions. If more than one bath is necessary while boarding, we will notify you at the contact number listed.

  • Please be aware that all boarding dogs will be walked three times a day Mondays through Saturdays and twice a day on Sundays.

    In case of illness or injury, I, the undersigned, do hereby give my authorization and consent for the staff of East Marietta Animal Hopsital to treat, prescribe for, or perform surgery on my pet(s) while they are being boarded at the hospital. I also understand that every effort will be made by East Marietta Animal Hospital to contact me by phone prior to performing any services that are necessary due to illness or injury.

    East Marietta Animal Hospital, its doctors and staff will use all responsible precautions against illness, injury, or escape of any boarded or hospitalized pet(s), but they will not be held liable or responsible in any manner whatsoever, under any circumstances, on account of the care, treatment, or safe keeping of my pet(s), as it is thoroughly understood that I assume all risks.

    Should the circumstances arise that my pet(s) remain unclaimed after the scheduled pick up date, I understand that written notice will be mailed to the address listed on the pet(s) medical record notifying me of such case. Fourteen days after such written notice, the pet(s) will be considered abandoned and may be taken to the Cobb County Humane Society. It is further understood that such action will not relieve me of any costs incured while boarding and any and all costs incurred by East Marietta Animal Hospital as a result of pet(s) abandonment.

    I hereby declare that my pet is current on all immunizations required for boarding and that my pet(s) are free of internal and external parasites. I also agree that the required immunizations have been given within the last 12 months. If I cannot show written proof (medical record from previous veterinarian with name, address and phone number of veterinarian) of such immunizations, I hereby give East Marietta Animal Hospital permission to administer any and all required immunizations and check for or treat for internal and/or external parasites at my expense.

    Boarder belongings are welcome; however, we are not responsible for personal items that are chewed or soiled by the patient. While boarding, the patient could become more excited than while at home, this could cause them to have a greater interest in their belongings which can result in an increase of chewing, etc.

    • With personal items, please note we need to label the items with the patient's name to ensure they are returned to the owner at check out.
    • At times it may be necessary to wash your pet's belongings. We will make every effort to be sure they are ready for you at the time of pick-up.
    • Please limit belongings - 1 blanket, 1 toy, and 1 snack

    Due to the additional time involved for administering medication to patients, pets requiring more than 2 medications daily will have a higher charge than pets needing 0-2 medications.

    • Diabetic pets needing insulin injections can have those medications administered for $10.00 per injection.
    • If your diabetic pet is not eating, glucose checks costing $16.50 each will be necessary to ensure your pet can have insulin administered.
    Playtimes are available for qualified pets at Leader of the Pack next door, please ask for details. I have read, understand and agree to the above written hospital policy.
